Here you will find electronic copies of official forms and documents used by Midland Adventist Academy. Click on the Download button to open or save a copy for future use.
New This Year!
All Registration documents for the 2010-2011 school year may now be filled out on-line, then printed for signing before bringing them in to Midland.
- When you open a document, the fields you need to fill in should be marked in blue. If they aren't, click the Highlight Fields button in the upper right.
- Right under this button is a Print button - use this to quickly print the page after filling it out.
- Depending on the level of Adobe you have available, you may be able to save a copy to your computer and fill it out later as you have the time. To do this, click File/Save As. (if you don't see that option, then you can't save)
Best of all, if you submit your application before July 9, you'll save $50.00 off the cost of tuition!
BIG HINT: If you have more than one student at Midland, fill out the first application completely and print (or save) it. Then change only the parts that differ for your additional student(s) and print (or save) that copy. You'll save yourself a LOT of typing!
Note: All documents are in PDF format unless specified otherwise. The Registration packet MAY require you to upgrade to Adobe Reader version 8 before opening.

Registration packet - This contains the first five documents listed below, and will save you some time when downloading all the forms. Note: The Health Inventory form is required for new students OR for students entering 4th grade and 9th grade

Student application - This is the primary document for Midland's student registration process.
|
|
|
Financial application - This contains information about the parents and/or those responsible for paying school tuition. Specific dollar figures are included for elementary, middle school and academy.

Emergency information - This contains emergency contact information, a consent-to-treat agreement, and a student-release section.
|
 |
Health inventory - This contains information on health history and a medical evaluation of the student. This form is required for new students OR for students entering 4th grade and 9th grade.

Image release - This form allows a parent or guardian to indicate whether or not their child's name and/or photo may be used by Midland Adventist Academy for news releases, advertising, website use, etc.

Community service - This form is used to report community service. High school students should complete 30 hours of service each year.
|
 |
Pre-arranged absence - This form should be filled out when a student will miss school for reasons including attendance at a funeral, doctor appointments, personal illness, or unique family situations for which prior arrangements have been made.

School handbook (2010) - This contains school history, goals, requirements for graduation, and numerous other helpful articles and school rules. If you want the straight scoop on Midland, this is where you'll find it.

Corral photo book - This yearly booklet contains pictures and quotes from all of our students and faculty.

Student directory - This yearly book contains addresses and phone numbers for our students, faculty, and other relevant areas.
