This page contains electronic (PDF) copies of official forms and documents used by Midland Adventist Academy. To open or save a copy for future use, click on the image in the bookshelf or select the Download button.

All Registration documents for the 2017-2018 school year (with the exception of the Financial application) may now be filled out on-line, then printed for signing before bringing them in to Midland.
  • When you open a document, the fields you need to fill in should be marked in blue. If they aren't, click the Highlight Fields button in the upper right.
  • Right under this button is a Print button - use this to quickly print the page after filling it out.
  • Depending on the level of Adobe you have available, you may be able to save a copy to your computer and fill it out later as you have the time. To do this, click File/Save As. (if you don't see that option, then you can't save)

Best of all, if you submit your application before July 15, you'll save $50.00 off the cost of tuition!

BIG HINT: If you have more than one student at Midland, fill out the first application completely and print (or save) it. Then change only the parts that differ for your additional student(s) and print (or save) that copy. It'll save you some typing!
